We work carefully to be ready for your appointment on time.

On your first visit, we'll need you to fill out a Medical History form. You can print it now to save time. Please complete it and hand it to our receptionist when you arrive. (If you aren't able to print it for some reason, no problem -- we'll have the form ready for you at reception.)

Our Satisfaction Survey is also available here. If you didn't fill it out after your visit, please complete and mail it to us as soon as possible. This will help us make sure we are providing the best experience possible for you and other patients in the future.

To view or print these forms, the free Acrobat Reader program is needed. Your web browser probably has it already installed. If not, you can get it here, no charge.

To save a form on your computer:
(Windows) Right-click on "View / Print / Save", and choose "Save Target As..." from the menu that pops up. Remember where you saved it.
(Macintosh) Press control key + click on "View / Print / Save", and choose "Download Linked File" from the menu that pops up. Remember where you saved it.
